    Notes:

    Op 18 december 1919 krijgt hij een paspoort. Uit zijn aanvraag blijkt dat hij sinds 1845 in de VS is en dat hij in 1856 is genaturaliseerd. Hij heeft sindsdien continu in de Verenigde Staten gewoond en wel in Ohio, Missouri, Iowa en Colorado en woont nu in Denver, Colorado. Hij heeft tijdens de Amerikaanse Burgeroorlog gediend in het legeronderdeel van Missouri en is in 1864 of 1865 afgezwaaid.

    Hij was eerst bedrijfsleider van de machinefabriek van Hendrie in Burlington, Iowa. Toen het bedrijf naar Denver verhuisde ging hij mee. Samen met Hendrie richtte hij de 'Hendrie & Bolthoff manufacturing & supply company' op. Dit bedrijf werd erg succesvol en leverde mijnbouwmachines door het hele land.
